Pathology report
The biopsy report for each lesion being referred, including the anatomic site as labeled by your office.
Photos of the biopsy site
A clinical photo taken at the time of biopsy helps confirm the site on surgery day.
Patient demographics and insurance
Current contact information and a copy of the insurance card so our staff can reach the patient and verify coverage.

What Happens Next
Dr. Lopez personally reviews each referral, and our staff promptly reaches out to the patient to schedule. We treat the patient, then write to your office so you know exactly what was done.
Most patients have their consultation, tumor removal, and reconstruction completed in a single visit. Dr. Lopez performs 99% of his own reconstructions, including local flaps, full-thickness skin grafts, and multi-stage repairs, so there is no separate surgical team to coordinate.
What Your Office Receives
After treatment, your office receives a cover letter from Dr. Lopez with a copy of the clinical note and photos.
When pathology is pending after the visit, we hold the correspondence until the report is back so your office receives the complete picture at once.
Your patients return to you for their ongoing care.
Cases Dr. Lopez Accepts
Dr. Lopez accepts referrals from dermatologists, primary care physicians, plastic surgeons, oculoplastic surgeons, facial plastic surgeons, and radiation oncologists across Tampa Bay. He has personally performed over 10,000 Mohs procedures since completing his fellowship at Mayo Clinic in 2018, including 1,757 in 2025.
Referred cases include basal cell carcinoma and squamous cell carcinoma of the nose, eyelid, lip, ear, and scalp, large or recurrent tumors, and tumors requiring multi-stage reconstruction. Slow Mohs with permanent sections is available for melanoma in situ and select early invasive melanomas.
